سندرم سروتونین یک وضعیت بالقوه تهدیدکننده حیات است که می‌تواند در نتیجه مصرف درمانی داروهای سروتونرژیک یا تداخل دارویی رخ دهد. در این مطالعه، دو مورد بحران فشار خون همراه با سندرم سروتونین پس از مصرف لینزولید توصیف می‌شود. بیمار اول یک خانم ۵۲ ساله بود که به دلیل عفونت پای دیابتی و پنومونی بستری شد. سندرم سروتونین ۲۴ ساعت پس از شروع لینزولید رخ داد. فشار خون مقاوم، یافته اصلی همودینامیک بود. فشار خون مقاوم و سایر علائم سندرم سروتونین حدود ۴۸ ساعت پس از قطع لینزولید برطرف شد. مورد دوم مردی با سابقه پیوند کلیه، دیابت و فشار خون بالا بود. ۲۴ ساعت پس از شروع فنتانیل و لینزولید، بی‌قراری شدید، کلونوس چشمی، هایپررفلکسی، فشار خون بالا و تاکی‌کاردی مشاهده شد. با تشخیص احتمالی سندرم سروتونین، فنتانیل قطع و مورفین شروع شد. علائم بیمار ۴۸ ساعت پس از قطع فنتانیل بهبود یافت. ارزیابی کامل سابقه پزشکی بیمار می‌تواند به پزشکان در پیشگیری از این سندرم در بیماران بدحال کمک کند.

چکیده (انگلیسی):
Serotonin syndrome is a potentially life-threatening condition that can occur as a result of the therapeutic use of serotonergic medications or drug interaction. In this study, we describe two cases of serotonin syndrome-associated hypertensive crisis following Linezolid use. The first patient was a 52-year-old female who was admitted due to a diabetic foot infection and pneumonia. Serotonin syndrome occurred 24 hours after starting linezolid. Resistant hypertension was the main hemodynamic finding. Resistant hypertension and other symptoms of serotonin syndrome were resolved about 48 hours after discontinuation of linezolid. The second case was a man with a history of kidney transplant, diabetes, and hypertension. Within 24 hours after fentanyl and linezolid initiation, severe agitation, eye clonus, hyperreflexia, hypertension, and tachycardia were noted. With the possible diagnosis of serotonin syndrome, fentanyl was discontinued, and morphine was initiated. The patient's symptoms improved 48 hours after discontinuation of fentanyl. A thorough evaluation of the patient's medical history can help clinicians prevent this syndrome in critically ill patients.
